Choosing between metoidioplasty and phalloplasty is one of the most important decisions in trans-masculine genital surgery. Goals often include standing voiding, sensation, aesthetics, number of stages, and recovery time.

Metoidioplasty uses testosterone-enlarged clitoral tissue to create a neophallus, often in one stage with urethral lengthening and scrotoplasty. Phalloplasty typically uses a flap (such as MLD or abdominal flap) for greater length and girth, with additional stages possible for erectile devices.
